We are seeking a highly skilled and motivated Burden of Illness New Practice Project Manager who will play a critical role in integrating new primary care providers and practices into Millennium Physician Group. This individual will be responsible for overseeing and optimizing the Medicare Risk Adjustment (MRA) performance within the new practice. The project manager will serve as the primary liaison, ensuring that data conversations, provider education, engagement, and performance tracking align with organizational goals.

This role involves effective project planning, technical support, and stakeholder communication, ensuring that all projects and applications meet quality and timeliness standards. The ideal candidate will possess strong leadership capabilities, operational acumen, and a proven track record in project management.

Responsibilities

Project Management

  • Develop and execute a strategic integration plan for the new primary care practice, with a focus on Medicare Risk Adjustment performance.
  • Manage timelines, resources, and deliverables to ensure project goals are met.
  • Provide regular updates to leadership on project status, challenges, and outcomes.

Medicare Risk Adjustment Monitoring

  • Oversee and report on the MRA performance metrics, identifying trends, successes, and areas for improvement.
  • Collaborate with the data analytics team to ensure accurate and timely reporting of MRA-related metrics.
  • Analyze performance data to identify gaps and develop actionable strategies for improvement.

Provider Education and Engagement

  • Work closely with providers to ensure they understand MRA processes and requirements.
  • Coordinate with BOI educators to deliver tailored training sessions to enhance provider performance.
  • Foster strong relationships with providers to drive engagement and align with organizational goals.

Opportunity Identification

  • Conduct regular assessments to identify the greatest areas of opportunity for MRA optimization.
  • Propose innovative solutions and process improvements to enhance performance.

Collaboration and Communication

  • Act as the primary point of contact between the new practice and internal BOI team.
  • Ensure all stakeholders are informed of progress, challenges, and best practices.
  • Facilitate effective communication between clinical and operational teams.

Qualifications

  • Bachelor's degree in Healthcare Administration, Business, or a related field
  • Minimum of 3 years experience in project management
  • Strong analytical and problem-solving skills, with the ability to analyze complex situations, learn quickly and synthesize corresponding solutions, options, and action plans.
  • Exceptional communication and interpersonal skills, with the ability to build rapport with diverse teams.
  • Proven ability to manage multiple projects simultaneously and meet tight deadlines.
  • Ability to successfully work in fast-paced, multi-tasking, fluid environment with ever-changing priorities and has the ability to foster a collaborative working environment with all staff levels across the organization.
  • Knowledge of Value Based Care program initiatives, preferably with experience related to Medicare Risk Adjustment applications and workflows.
