Job Description:
Evaluates, designs, and implements individual programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients. Performs and documents thorough initial evaluations, identifying problem areas, contraindications to treatment. Respects patients' rights of confidentiality and privacy and complies with HIPAA regulations. Delegates and provides instructions to Physical Therapy Assistant.(where applicable) Adheres to policies and procedures of Millennium Physician Group, abides by Standards of Ethical Conduct for Physical Therapists as established by the American Physical Therapy Association and by rules and regulations of the Florida State Practice Act, Chapter 486.

Job Standards:
Establishes achievable, functional and measureable goals
Provides instructions and education to patients, family members and care givers
Communicates effectively with physicians and other health care providers involved with the care of the patient
Documents initial evaluations, assessments, plans of care, daily encounters, progress reports and discharge summary in timely manner
Works with office personnel to ensure timely scheduling of patients, insurance coverage and flow of daily operations

Qualified candidates will be a Florida licensee. New grads are welcome!
